Green County – The week of May 10th, 2021 Green County Public Health will be offering 3 pop-up COVID-19 vaccine clinics across Green County.
- Brodhead, Monday, May 10th, 11am-1pm
- Methodist Church (501 1st Center Ave, Brodhead, WI 53520)
- New Glarus, Thursday, May 13th, 10:30am-12pm
- New Glarus Bible Church (207 6th St, New Glarus, WI 53574)
- Monticello, Thursday, May 13th, 12:30pm-2pm
- “Old Village Hall” (140 N. Main St. Monticello, WI 53570)
These clinics will all be free and individuals do not need to make an appointment. No ID, health insurance, or proof of residency is required to get the COVID-19 vaccine.
The Johnson & Johnson (Janssen or J&J) vaccine will be provided at these clinics. This a single-dose vaccine approved for people 18 years of age and older. Individuals who attend this clinic will need to complete a vaccine administration form, which can be found on the Green County Public Health website. Individuals should print and complete this form, and bring it to the clinic. This form will also be available on-site at the clinic for those who do not have access to internet/a computer. Individuals should also review the Johnson & Johnson COVID-19 vaccine Emergency Use Authorization (EUA) fact sheet, this can be found on our website and will be available on-site.
Green County Public Health is also offering walk-in COVID-19 vaccine clinics every Monday from 1pm-3pm and every Friday from 9am-12pm at the Green County Government Services Building (N3152 WI-81, Monroe). The Moderna COVID-19 vaccine is being provided at these clinics, which is approved for people 18 years of age and older. The Moderna COVID-19 vaccine requires a second dose 28 days after the first dose. Individuals can visit vaccines.gov to find other local COVID-19 vaccine providers near them.
